PROOF OF RESIDENCE DOCUMENTS
Players must provide 3 different categories of proof of residency. Copies will be maintained in the player’s folder while he or she
is an active player with no break in playing season. Once a player has a break in season (does not play one season)
he or she then must again provide 3 proof of his or her current residency to re-register.

Report Cards, Progress Report, Registration, or anything that shows it was sent from the school and has both the players name and address on it. |

State records |
Anything issued by a State that has the players address and the family name on it. |

Anything Local (City or County) that has the players address and the family name on it. |

Support Payments |
Self explanatory |

Homeowner's or Tenant Records |
Escrow papers, hone lone, paperwork or statement, property taxes |

Utility Bills |
Gas, electric, water/sewer, phone, mobile phone, heating, waste disposal or anything else that provides service into or for the home. |

Financial records |
Loan, credit, investments, etc. |

Insurance documents |

There are only two exceptions...rules 2d and 4d. A player that lived within our boundaries and has moved out but continuous
to play with our league with no break in playing season qualifies under the 2d rule. The 4d rule is for an adult wavier
who volunteers within our league for two years and continues, his or her child is eligible to play.
